    Hi Colleagues
    Do you know of infection prevention pamphlets / brochures / posters developed in conjunction with aboriginal communities? The focus would be on preventing health care associated infection rather than childhood immunisation.
    I’m seeking something that could be use in this region in a modified form as necessary.

    Dear Craig,

    Our District Uniform and Standards of Dress Policy states: Staff are to comply with any Facility/Service requirements to be bare below the elbows. It also states:

    Skivvies/long sleeved t-shirts are not to be worn underneath uniforms/scrubs.

    Most facilities do not use the terminology Bare Below the Elbows but do have inclusions in their HH policy around not wearing jewellery when providing direct patient care. Although facilities may not refer to this in their policies it is seen as best practice and wards have readily adopted the short sleeves (or rolled up) and no lanyards. Many medical staff have opted not to wear ties particularly those more involved in direct patient care.
    We are fortunate to have a Senior AMO who champions short sleeves, no ties, no lanyards and hand hygiene.

    Hi,

    Hundreds of nervous medical registrars are about to fan out all over Australia for their Royal Australasian College of Physicians exams from July 25 to August 3, as they do every year. In hot pursuit will be about 150 “National Exam Panel” (NEP) members, of whom I am one. If you are wondering, it definitely is “all beer and skittles”.

    Some of you can relax…I ditched my tie years ago, my jacket is left hanging on a chair, sleeves are rolled up, my pedometer has a watch, and there is no lanyard to be seen. For many others though, it is all about the grandest suit and tie they can conjure. Of interest, it’s amazing to see how easily those ties flop on to patients when candidates lean forward, but I digress.

    I take it as a given that you all practice the 5 moments and cleaning of reusable equipment between patients. Do any of you in public (or some larger private) hospitals have a dress code (eg, BBE) that we NEPs and exam candidates must respect? Further, is it guidance (“should”) or actual policy (“must”)? Feel free to send on to me.

    Hi Wendy,

    I have been informed that some institutions have been autoclaving their
    own labels. However, there are manufacturers (eg Mayo Health Care) who
    have the labels pre-steriled including the pen. The product is Triac
    sterile labelling system. Toni.

    The question has been raised re the new standards for labelling of
    medicines.

    Has anyone developed a process for labelling injectable medicines when
    administering them as part of a sterile procedure e.g. connecting to a
    haemodialysis machine.

    I’d be very interested to know what protocols are in place in Australian
    hospitals where breast milk donor banks exist. Our neonatal unit is in
    the process of setting up a bank and all milk will be pasteurised;
    however, I would like to know what advise is given to mothers regarding
    the cleaning or disinfection of the breast pump equipment at home.

    NSWHD Infection Control Policy 2007-036 states:
    “Domestic type washing machines must only be used for a patient’s
    personal items. Washing must involve the use of an appropriate detergent
    and hot water. If hot water is not available, then only individual
    patient loads can be washed at one time. Clothes dryers should be used
    to dry a patient’s personal items. Used/soiled linen must not be rinsed
    or sorted in patient care areas or washed in domestic washing machines.”

    Refer to the Australasian Health Facility Guidelines, Part D- Infection
    Prevention and Control http://www.healthfacilityguidelines.com.au/
    This document provides information on the four different types of basins
    required in a hospital and their specific uses. Also the AHFG provide
    room layout ‘standard components’ sheets which also tell you what type
    of hand basin is required including its purpose.
    Although you may not be undertaking any renovations / building works the
    infection prevention and control section is still sound advice and is
    mandatory for NSW (it is listed in the front of the NSW Health
    PD2007_036).
    I can’t help you on the laundering of patient clothing as we
    specifically removed all patient washing machines and clothes dryers a
    number of years ago.

    We have a designated elective ward where patients are screened
    pre-admission. If a patient needs to be admitted immediately due to
    their medical condition to this ‘ring fenced’ ward they are placed in a
    single room and swabs collected for MRSA. “Released” once proven
    negative.
    We have another ortho ward in our main hospital where known MROs can go
    and trauma cases. In this ortho ward the patient would not necessarily
    be placed in a single room unless the organism was significant.

    Just some quick questions about orthopaedic patients with infections.

    1. Do you accommodate orthopaedic patients with active wound infection
    on your post-op orthopaedic wards?
    2. In particular, do you accommodate orthopaedic joint surgery patients
    with active wound infections on the same ward as post-op orthopaedic
    joint surgery patients?
    3. Do you allow patients colonised with MRSA or ESBL’s to be
    accommodated on the same ward as post-op orthopaedic joint surgery
    patients?
    4. If yes to any of the above, are patients on orthopaedic wards with
    colonisation or infections always placed in single rooms?

    These are contentious issues for orthopaedic surgeons in our facility
    (and probably many other facilities!), so any comments or thoughts on
    risks and risk management strategies to prevent joint infections
    post-operatively would be appreciated.

    Does any one know of any useful published guidelines about this?

    Hi Fiona,
    We have placed hand foam product at the entrance to each ward (inside at
    patient entrance and outside at staff entrance); outside each room and
    at the foot of each bed. Outside rooms are 1 litre lockable and at then
    end of the bed 400ml (as with Giulietta can be turned to lock). In our
    OPDs we have a gel (probably change to the lockable foam dispenser
    eventually) that is generally in view of staff. Outside the cafeteria
    is a lockable foam dispenser and at the main visitor lift is a lockable
    metal stand dispensing foam.
