    Hello

    Just hoping for quick survey for those site who have an ICU and the advice from the ACSQHC Table 2 – high risk units re MRO screening on admission , weekly and on discharge (especially VRE)
    as I am getting mixed messages from one of our ICUs that other units no longer undertake weekly MRO screening despite the national guide so I am trying to get an understanding of what others unit may be doing

    Question – Do you currently screen ICU patients for MROs on weekly as well as on discharge and admission

    Hi All

    We organise a re-test with a different tester on a different day. Several of our staff had successful tests the second time around. We add staff who still do not achieve a fit to a spreadsheet for subsequent follow up in case different masks become available. The staff member is supported to not work in any area that requires them wearing and N95/ P2 respirator.

    Thanks. It has been reported to TGA. There appears to be at least 2 batches of vaccine and 3 batches of needles involved at this stage.

    Hi Sue,

    You may have already done this but if not your issues/problems should be reported to the TGA using the Medicine or defective vaccine report form at the following link:
    https://www.tga.gov.au/medicine-or-vaccine-defect-report

    As per TGA – The report is to report defects that you think have arisen during manufacture, storage or handling of medicines. These sorts of problems are usually found in a single batch or a single pack of a product. These problems may require investigation by the Therapeutic Goods Administration (TGA) Laboratories.

    Your particular issues/problems may relate to a specific batch.

    Hi All,

    I would like to ask if anyone else is having issues with the AfluriaQuad Influenza vaccine and BD Eclipse needles. We have had a number of leur lock mechanism breakages as well as vaccine leaking requiring a 2nd dose. We have also had a needle stick injury post vaccination when the needle and syringe fell apart as the safety mechanism was being activated. I also have been informed that another organisation have had 3 needle stick injuries within their IC department using the same combination of vaccine and BD Eclipse needles. I have attached a photo of the broken syringe / needle combination.

    This issue has been reported to TGA, BD and Seqirus

    Good Morning Everyone

    Peninsula Health use a database that links to our HR system which can generate reports on

    Each Cost centre or whole organisation

    Staff category e.g A, B, C

    Compliance e.g completed, in progress, medical exception, allergy etc.

    Vaccine details e.g date of last Pertussis

    Immunity details for Measles, Mumps, Rubella, Varicella and Hep B

    Can also generate individual immunisation certificates with details of vaccines given at PH and last influenza vaccination date

    The database started as an access database which I designed for purpose. It has taken quite a few years but it is now quite functional although some manual entry is still required.

    At this stage we are focusing on getting staff to complete the form and document assessment each shift in the first instance. If compliance is good we may consider it in the future but not at this stage

    There is Cochrane evidence to support clinically indicated removal of PIVs compared to routine replacement are there any plans for the future to move to clinically indicated removal?

    Good Morning to you all,

    Our current Peripheral Intravenous Cannulation policy states; to resite a IV cannula every 72 hours. I know there is some debate on this issue and recent evidence suggests routine resite is unnecessary. Current recommendations are to resite IV Cannulas every 96 hours with the exception of children and patients with poor veins. Saying that, not all Hospitals has IV teams to resite all the necessary IV cannulas.

    Could you please let me know if any of your organizations have an IV Team and when do you routinely resite patients cannulas?

    Hi Michael,

    We also have an electronic alert system and include a similar comment re
    contact precautions only being required if the patient is symptomatic.
    We also ask for the patient to be on a bowel chart and have a prompt for
    discretion when prescribing antibiotics.

    We place an electronic alert in our system for patients identified with
    C. difficile infection and include a comment that contact precautions
    are only required on re-admit if the patient currently has diarrhoea or
    has had diarrhoea in the previous 48 hours.

    Moving to a new facility is always interesting, as things are done
    differently. Now I get to challenge my own thinking!

    Can I ask if any facilities routinely place into single rooms on
    readmission patients who have had previous Clostridium difficile
    infection (CDI)?

    I cannot find this supported in the 2010 SHEA/IDSA Clinical Practice
    guidelines (
    http://www.cdc.gov/HAI/pdfs/cdiff/Cohen-IDSA-SHEA-CDI-guidelines-2010.pd
    f), but then again it is not specifically mentioned (apart from
    screening asymptomatic patients and staff not being useful).

    Does any facility have a process for identifying patients in future
    admissions who had previously had CDI, and managing them differently
    regardless of status of diarrhoea??

    Thanks for any thoughts on this.
