I believe this utilizes warmed medical grade air. Please ensure IQ, OQ and PQ is done at commissioning and annual PQ and preventative maintenance in place.

I have been approached regarding introducing a new technology for endoscope reprocessing. This device enables fast scope channel drying and storage using “activated air”. The scope is stored in a dedicated sealed bag which can also be used for transport with barcode traceability. The facility currently does not have HEPA filtered forced air drying cabinets and sees this new technology as a possible alternative process to meet GESA/GNECA and AS:4187 compliance.
The device has recently been granted a ATG certificate.
I would love to hear thoughts from anyone who has also reviewed this product. Happy to provide product name off line.

We have to rely on recommendations of AS 4187:2014. Individual health service organizations shall write their own policy towards managing identified risk associated with this. We are fortunate in NSW to have not experienced this, as far as I am aware. Below are parts of the Standard that could be beneficial.
* 5.6.1: Shall provide the physical environment and equipment necessary…..this includes the requirements for environmental control…e.g…..humidity, air flow…
* 9.5…processed RMDs shall be stored in a manner that prevents/minimizes the risk of contamination….
* A9.5(g) RH should be controlled within the range 35%-70%

We do monitor our environmental conditions on all sterile stock areas. “Out of range” readings are reported immediately and our facilities management application software and does its job. This means an automated log of service providers is generated in an email to “fix” associated issues.
I don’t believe there is any need to have another guideline/document. If we have the standards recommendations, it is incumbent upon individual facilities to comply in maintaining what is required, in this case RH- 35%-70%.

Does anyone have guidelines for managing high humidity events in CSSD sterile store areas?
We have found that there seems to be have been an increased number of extreme heat and humidity weather events experienced in Victoria over the last two summers causing issues in sterile store areas and operating rooms in some of our health services.
There seems to be a lack of clear advice on what to do should the readings exceed the acceptable humidity levels in healthcare sterile storage areas. How long can items be exposed to humidity levels outside the recommended range and at what point should action be taken? That is, reprocessing of reusable sterile stock or disposal of commercially processed single-use medical devices exposed to high humidity.
Given the high cost of reprocessing reusable medical devices or disposal of single use medical devices it would be very useful to have a consensus document to guide actions in such an event.
1. Are other health services experiencing high humidity events?
1. Is the humidity level in your CSSD sterile store area monitored and recorded daily?
1. Do you have an existing policy/procedure for managing high humidity events in CSSD sterile store areas?
1. Do you think there is a need for a contemporary Australian document guiding action in a high humidity event?

In addition to the guidance compliance to the AS 4187 is required. It must be emphasized as well that this limitation is subject to compliance with:
5.6.10 – cleaning of facility (including associated equipment – re storage cabinets) to maintain hygienic conditions at all times
5.6.14 – Ventilation in storage areas must comply with AS 1668.2
10.3.1 – cabinets with filters (we have one for our probes storage cabinet) must be checked and changed regularly.These parts ensure integrity of processed item prior to patient reuse, expiry use by dates taken into account.

Just seeking clarification as to whether the storage cupboard for a processed TOE probe requires HEPA filtration.
Regarding storage, the current ACIPC / ASUM guidelines (2017) state :
“after cleaning, all transducers must be stored in an appropriate environment to protect from environmental contamination (AS/NZS4187:2014, Table 5.1). It is recommended that a specific cabinet is used, but if this is not available the minimum standard recommended is a clean disposable cover applied to the transducer to mitigate risks from environmental contaminants”
So I gather as HEPA filtration is not mentioned, that it is not a requirement. Is my understanding correct?

Hi all
First we have reusable (reprocessable) blades with single use disposable ones as options on high risk patients e.g. +TB. Yes we are able to track as a basic requirement of compliance to Standard 3.17 and applicable parts of AS 4187:2014 (2.2(m); 2.4.3 and 9.4). Single use items have labels you can attach to patient medical records (scanning in cases of digital applications of patient management system as applicable).
Second, ensure preventative maintenance schedule is in contract post device warranty. Usually we have a year warranty of functionality; it would be beneficial to have a preventative maintenance contract. This will provide regular assessment of functionality of your devices. It is also included in AS 4187:2014 – 2.2.3(k); 5.1.3; 6.4.2.

Hi all,
We are interested to know how other organisations are managing their laryngoscope blades. This is something that we are currently looking at.
A couple of questions;
Firstly, are most organisations using disposable or reprocessable blades? Secondly, if you are reprocessing blades, are you able to track them to a patient i.e. do clinicians put the sticker in the notes?
And finally, particularly for resus areas, do you find that clinicians leave blade packets open in order to check the lights regularly? How are you managing this as a contamination risk?

EN285 please. If you are talking about the need to have a pass though steriliser then I can only have this:
* AS 4187:2014 – 5.6.2 only emphasized the need to have a unidirectional workflow from the dirty to clean areas. In this context, the integrity of the processed item is at greater risk of recontamination and best practice suggests to have a pass through if it is feasible. It is not explicit though,

“The size of the area will be dependent on the number and type of sterilizers installed. Pass through sterilizers are assumed as this provides the necessary separation of loading and unloading zones within the sterilization area”Hope this helps

Just wondering if I’m in the wrong here thinking that sterilising equipment bought from the local hardware actually isn’t appropriate? http://www.abc.net.au/news/2017-03-03/bega-hospital-left-with-one-orthopaedic-surgeon/8322442 Sure I wouldn’t want to transfer the patient, but I see this as a very slippery slope.

Our facility are in the process of planning for a hybrid theatre, I would like to know if there are any standards written for them? Also has anyone been involved in the planning and completion of a hybrid theatre? If so do you have open ceiling control rooms?
Our team are requesting that a 2.1 meter wall (with glass)door free is built for the control room instead of floor to ceiling wall. There reason being it’s easier to communicate with the surgeon!!
Thus far I have said NO to the 2.1 meter wall hence my question here today. I’d be happy for any advice.

Correct me if I see it otherwise. This is simple Spaulding non critical item. Decontamination is suffice. It is of equal importance to require the manufacturer of instructions for reprocessing and establish functionality of the device when exposed to the rights of detailed decontamination. If it can withstand thermal disinfection (anaesthetic cycle) would advise to do that. It is important to preserve the integrity of the processed item as exposure to a historical microbial medium is at play.
I understand the limitations of operational budgetary constraints, but I would endeavour to at least HLD. I hope this helps.I did work at a place that sterilise them at 121C.

Thanks for the query.
As applicable to the specific reprocessing equipment in use in the facility, relevant current standard in particular to those cited as normative references ( which is technically a part of the AS 4187:2014) should be available. Due to enormous of content of a whole standard which is not applicable, it is suffice to have the parts cited to be purchased alone, whereas others are required in whole. The required is enumerated in Section 1.3 of the standard. The impression is that information is available to comply with its requirements of compliance. So yes, you are only required to have reference material relevant to compliance and for those equipment/devices that is applicable to your facility.

Are you saying that you can purchase specific sections of an ISO standard (i.e. the normative component of the compliance) rather than the whole standard and that this section includes specific details to enable auditing of a facility?
You mentioned independent organisation/s (comes with a price) that you can purchase audit tools from – can you provide the details and ill get some costings.

I am lucky that the facility I work with happened to have an ISO accreditation 3 weeks ago. My role have given me the option to comply with some of the requirements of the AS 4187:2014.

Please be aware that you are only required to access parts of the relevant standards specifically referred to as a normative component of the compliance. I have known managers in areas where they have purchased what is required.

Answer: I wouldnt worry much about the cost of the ISO standards. It would be more on the equipment compliance, monitoring and contingencies attached to gaps if the required monitoring suggests resolution of issues ( how do you manage them?). An example is specific to Table 7.2 where are the requirements of water quality, endotoxin, among others are specified. Also too the AS 4187:2003 is referring to “older” AS standards compliance whereas the AS 4187:2014 refers to a more current and ISO (at that level) which is more stringent and technically demanding, so there are, unfortunately dollar values involved as with other compliance.– have organisations been able to develop an audit tool that include standards (i.e. ISO standards) that are referred to in AS4187:2014?

In relation to questions posted below. I will try my best to answer. Otherwise I can be contacted on my work email. Happy to help. I am lucky that the facility I work with happened to have an ISO accreditation 3 weeks ago. My role have given me the option to comply with some of the requirements of the AS 4187:2014.
– have organisations been able to access/obtain all the standards that you are required to meet in AS:4187:2014 (i.e. AS and ISO standards)? – Answer: Yes at the facility I am currently employed and at a previous facility I worked with. Please be aware that you are only required to access parts of the relevant standards specifically referred to as a normative component of the compliance. I have known managers in areas where they have purchased what is required.
– what was the cost associated with obtaining these standards (i.e. ISO standards)?
Answer: I wouldnt worry much about the cost of the ISO standards. It would be more on the equipment compliance, monitoring and contingencies attached to gaps if the required monitoring suggests resolution of issues ( how do you manage them?). An example is specific to Table 7.2 where are the requirements of water quality, endotoxin, among others are specified. Also too the AS 4187:2003 is referring to “older” AS standards compliance whereas the AS 4187:2014 refers to a more current and ISO (at that level) which is more stringent and technically demanding, so there are, unfortunately dollar values involved as with other compliance.– have organisations been able to develop an audit tool that include standards (i.e. ISO standards) that are referred to in AS4187:2014?
Answer: There are independent organisation/s (comes with a price) that have prepared but I am not aware of the sufficiency of the audit. It would be interesting if the college would fund such an audit tool which I think many of us will be happy to be part of.Roel Castillo
