For us, it would depend on what type of isolation you are exiting the room from. If it is droplet precautions, then as long as you are 2+ metres away from the patient, you can safely doff the mask inside the room as you leave it.
If it is airborne precautions, you cannot doff the mask until you have left the room and closed the door (even if there is a anteroom).
In the case of COVID-19 or suspected COVID-19, we are utilising airborne precautions currently, so you would only doff a mask outside the room, after the door had been closed.
It is all about risk of exposure to the respiratory pathogens you are isolating for.

Subject: [ACIPC_Infexion_Connexion] Mask doffing (in or out of room)Hello All You Clever People
I am trying to provide a rationale re doffing mask after you have exited a residents room when the resident is in isolation.The person insists that you remove ALL PPE whilst in the infectious room. In aged care there is no ante room for doffing so you are either in an infectious environment or outside the room.
I have tried to explain that you doff all PPE except for mask in the residents room and remove mask when outside their room.
Can anyone point me to a document that says this – or am I wrong?

NSWThanks to everyone who replied to my request for help with this issue.
I must admit I was surprised the surveyor took this approach, as she has a background in infection control, but when I further discussed it she did note it was ACHS’s interpretation, not hers.
She was also very happy with my risk assessment and plan to manage sterile stock in ward store rooms, so we won’t have to change our shelving order, which was a relief!

living through accreditation this week (my nightmare) same issues!!!
We have scopes that are stored in a vented cupboard that have high level disinfection, yet the surveyor states we have to have them in a airdrying cupboard. The standard doesn’t state that for endoscopes, the GENCA guidelines do though (but they are a guide). We are in the start of major rebuild and in 3 years they will be in a pass through cupboard that meets all of GENCA guidelines! We test our scopes, and we also place an alcohol rinse through them before airdrying and storage hanging. We have a plan and have risk mitigated…I don’t think there is anything more we can spend our $$$ until we move.
I can’t wait until we actually have accredited Standard 3 credentialled ICP ( who truly understand the standard and all it entails) auditing the standard… my dream as a previous president and something I did try and follow through with ACHs.

The auditor is correct in that sterile storage for commercial sterile items has always been covered by ASNZS 4187 in the past but it seems to have got lost in the new 2014 edition .
ASNZS 4187 2003 Section 9.1.1 Sterile items states… “Sterile items shall be stored and handled in a manner that maintains the integrity of packs and prevents contamination from any source. This requirement applies equally to items sterilised in the health care facility AND to sterile items procured from commercial suppliers ”We also know that sterile items need to be stored in a ‘designated’ area so that rules out storage with unsterile items.
Sterile items can so easily be compromised if not in a DESIGNATED and restricted area
( so reduced traffic) with the correct ventilation and humidity etc. So as you know different storage is required for sterile items.
Unsterile items are so often not handled with care and are easily compromised. So we don’t want them stored with sterile items.Thanks

________________________________I’ve just had my mind blown by an auditor who tells me I cannot store any ‘sterile stock’ on a shelf with non-sterile stock. I reply by saying that no sterile RMDs are stored on shelves with non-sterile stock, and they say, no ANYTHING that comes sterile should not be on the same shelf with something non-sterile.
Have I missed something? I have always understand that the very specific storage requirements for RMDs did not apply to most commercially sterilised single use items. Sure, there are commercially sterilised disposables that have very specific storage requirements, but the majority of high volume disposable sterile goods can be safely stored on a shelf with non-sterile stock in an appropriately air-conditioned storage room. AS4187 does not cover storage of non-RMDs, correct?
Can someone either tell me I am wrong, and I missed this big time, or that I did not miss anything, and the auditor is incorrect. Or some variation of these.

That document is exactly what we are talking about, but nobody can show me where in AS4187 or the current NHMRC guidelines it says commercially sterilised stock cannot be on the same shelf as non-sterile stock. Obviously we don’t store sterile items on a shelf (or even in a room!) with food products (or cleaning products), but there is no real risk in storing a sterile disposable item like a disposable urinary catheter (that is appropriately packaged) on a shelf with non-sterile gauze swabs that is also appropriately packaged.
Maybe my issue is I haven’t fully outlined why we are commutable with this as a full risk assessment, although to me it is common sense. I spent way more time on ensuring we are compliant with the storage of sterile RMD’s and the water testing requirements parts of this document!

It was a ward store room, and the items commented on where things like disposable sterile urinary catheters on the same shelf as unsterile gauze packs.

Subject: [ACIPC_Infexion_Connexion] CSSD – MRSA/ VRE precautionsIt has been brought to my attention that I don’t have a procedure for CSSD staff when they are handling equipment from a patient with MRSA, VRE or potential COVID case.
Can anyone share some light on how the instrumentation should arrive to CSSD?
Currently, it arrives in a tub, but should we have a sign on to say contact precautions required?
Do staff need to wear PPE?Any help would be appreciated .

We have recently started to see the use of face mask brackets / inserts creep into use in our facility with staff wearing surgical face masks.
We have been unable to find any specific literature but from an infection prevention and control point of view we have a number of concerns including:
* Does it create a gap in the seal between mask and face?
* Does it potentially pose a risk as a pressure causing agent?
* Is removal of the mask in a manner that won’t contaminate still possible with the bracket in place?
* Is the bracket stored in a clean space?
* Is the bracket cleaned after use and before storage?
Does anyone have any references regarding these products or has anyone investigated and / or authorised the use of these products within their facility.

We are fairly tight on space at our Northside facility. We have dedicated shelves or wire racks for most bags of fluids in wards, and the supply department decants out of outer cartons in supply, then into transport tubs, then onto shelves/racks in ward store rooms.

The issue here is one of validation of storage conditions. A commercially available single use item has packaging that has been validated under certain conditions, which will include things like transport. Reusable items reprocessed and packaged within a facility are only validated for ideal storage (eg idea temp/humidity and minimal handling/movement on fixed shelving), so you cannot assume it will be still be sterile unless you encase it in something that protects it (like your transporter of your commercial reusable trays does. And presumably has validated).
I am old enough to remember the days loan sets for orthopaedic procedures used to be sent from facility to facility without being reprocessed again, and I can remember opening some trays in theatre with a patient prepped and almost vomiting due to the dried crud still on the instruments that had supposed been cleaned and resterilised at the last facility used at!
There is a major difference between validation of commercially packaged goods and validation of facility packaged goods as far as storage conditions go. AS/NZS:4187:2014 quite rightly requires facility reprocessed goods to be reprocessed and resterilised on arrival to a different facility, unless there has been validation of storage and transport conditions for the items in question.

Hi Brigid,
We outsource all our sterilising to another healthcare facility. We are a small rural health care facility that does minor day procedures. It was not possible to do sterilising onsite due to staff and other issues. We have contracts in place, and all our equipment is shipped an hard walled plastic containers. Part of the contract is the facility we outsource through must show us annual compliance to AS4187. Most of our products are single use though, and there is not large amounts of instruments going back and forth. I am not sure why you would resterilise when it gets to a destination if the packaging is intact and not damaged in anyway. I don’t know of any service that resterilises single use trays or sterile gloves??Kind regards,

Could someone please point me in the right direction for standards for transporting instrument trays and RMD’s between hospitals? I understand they must be reprocessed prior to use in the next hospital as sterility cannot be guaranteed. Though some staff members are saying if transported in sealed plastic dust covers it is ok to use. I disagree, though the 4187 standards are a bit generic in this aspect.
My belief was that if to be transported from one hospital to another, many factors needed to be taken into account including , transport vehicle, humidity etc. The hospitals are about 30mins apart.

NSWReply to my own post as a member of the supply industry to laundries has provided this response to be posted anonymously.
Whilst not familiar with this type of bag, if it has been through the wash process and they achieved the 4146:2000 standard they should be able to remove the bag safely from the machine.
I don’t see any additional PPE required than they would be normally wearing when transferring the clean linen from the washers to the dryers.

Dear brainstorm,
I am not too sure of laundry related IPC measures so, would like to seek your guidance.
Is it okay to use “Soluble Seam Laundry bags” for soiled or contaminated laundry instead of complete dissolvable bag in the in-house laundry?
These bags are not compatible with dryer therefore, laundry staff needs to remove the bag manually after completion of wash or before placing clothes/linen into the dryer.
If it is, then what is the PPE requirement for the laundry staff?
I would love to receive any advice.

We follow the clearance requirements advised by Queensland Health here:
The following criteria should be satisfied prior to certifying that a client has cleared an MRO:
MRSA, MRGN and ESBL producing organisms (other than CPE)
More than three months elapsed time from the last positive specimen
All wounds are healed, no indwelling medical devices present
No exposure to any antibiotic or antiseptic body wash for at least two weeks prior to screening
In the case of MRSA, no exposure to specific anti-MRSA antibiotic therapy in the past three months
Consecutive negative screens from screening sites (refer to Table 1 for required screening sites) on two
separate occasions. The screening swabs must be separated by a minimum period of one week. The
period over which these swabs are collected should not be less than a week, but may be over a period
of months
Health services may consider using the evaluation of a single set of screening swabs with a broth
amplification technique for clearance of clients. This process should be based on local factors and
agreements with local laboratories
Management of multi-resistant organisms guideline (health.qld.gov.au)Your state health department may have similar advice, and it would be worth discussing with whichever pathology provider you use as to an agreed process, as they should advise on collection on specimens for clearance.

We use a commercial ATP test swab on all of our TOE probes, which is read in-house on a reader. This is a surrogate for micro testing, but helps us identify remaining proteins on the probes after initial cleaning, which gives us an indication of the need to re-clean them prior to disinfection. What we have found, anecdotally, is that TOE probes used in routine elective TOEs in Cath Lab are rarely heavily contaminated, but TOE probes used during cardiac surgery in OT are often heaving contaminated.

Hi brains trust,
I’m wondering if any hospitals perform TransOesphageal Echocardiogram (TOE) scope swabs and what specific tests are you requesting please.
I have encountered difficulties with getting any of our pathology testing departments to run these tests, as they say that they are not NATA accredited to do so and cannot find any clear process or working processes.
If you are currently performing these tests, please can you tell me about the specific standard and methodology for running said test.
Your assistance is greatly appreciated.
