    Hi Terry

    Story time. Years ago, I worked in a facility that had brand new state-of-the-art imaging equipment installed in a cath lab procedure room. A renowned vascular surgeon started doing all sorts of grafts endoluminally in the HEPA filtered room.

    Then a gastroenterologist did an after-hours emergency colonoscopic procedure in the room (required the imaging) and it “hit the fan”, so to speak. The vascular surgeons rebelled, calling this an outrage.

    The ID physician who was called to literally step between the two parties (little tiny guy, actually, quite brave I thought :)) explained that as long as the procedures (colonoscopy and vascular) were not done at the same time in the same room, the risk to any subsequent patients approaches zero if standard cleaning and any required spot cleaning is performed. He was quite factual about this.

    So, my response would be along those lines. As long as environmental cleaning protocols are followed appropriately after all cases, there is no reason a procedure room or theatre cannot be shared between disciplines, even with those ”unclean’ gastroenterologists. 🙂

    My opinion, anyway. No evidence based review, though, sorry.

    Hi everyone,

    I have been asked the question – is it ok to perform endoscopy procedures in the same Operating Room in which ophthalmic procedures are also going to be performed?

    I have also been asked is it ok to perform cystoscopy procedures in the same room in which ophthalmic procedures are performed?

    Obviously I have my own opinion about this – but I am interested to hear feedback and or any directions to an evidence based review around this topic from my colleagues as it appears to be quite an emotive issue!

    Hi Terry

    I would suggest that the active ingredients in any wipe are much more important than the name / brand of a wipe. Thus I would suggest we still maintain discussion of active ingredients rather than brand name.

    It does raise the question: Which disinfectants are effective against filoviruses? though, doesnt it? Apart for sodium hypochlorite (bleach), are there any other recommended disinfectants or combinations effective in inactivating filoviruses? Is that what we are trying to do in disinfecting disposable gloves?

    More questions than answers from me, Im afraid, Lindy.

    Hi Michael,
    Given Ebola urgency, uniqueness, and common need of members, can you allow an exception in naming of disinfectant brands and types? The CDC list of EPA registered is very frustrating and not applicable in Australia.
    Linda, CDNA recommends 1,000-5,000ppm hypochlorite for cleaning and spills (dependent on blood presence) it is economical, readily available and effective.

    Disinfection of gloved hands using either an EPA-registered disinfectant wipe or alcohol-based hand rub between steps of taking off PPE.

    Will you be considering ABHR or a disinfectant wipeif a wipe what would you use.

    We are just trying to understand what specifically is in the EPA registered disinfectant wipe so we can match it with what we have TGA approved and available in Australia as there are many listed but not available in Australia

    We have a disinfectant wipe here we use for environmental cleaning (I know I cant name products on this forum) here but know there are other products. Can anybody send me what they are using off line and if it comply with the EPA list as recommended by the CDC (I know you are busy so a quick reply with just a name would be great!)

    Hope you are all travelling wellhuge body of work being done by us all trying to have consistency for staff and pt safety and calm another thesis for somebody in the making hey?

    President signs Executive Order; releases national strategy on antibiotic resistance
    The President signed an Executive Order and released a National Strategy for Combating Antibiotic-Resistant Bacteria [http://www.whitehouse.gov/sites/default/files/docs/carb_national_strategy.pdf ]. The Executive Order tasks federal agencies to develop an action plan to implement the national strategy and address recommendations from the Presidents Council of Advisors on Science and Technology (PCAST) [http://www.whitehouse.gov/sites/default/files/microsites/ostp/PCAST/pcast_carb_report_sept2014.pdf ] on ways to combat the threat of antibiotic resistance. An estimated 2 million Americans will contract an antibiotic resistant infection according to the Centers for Disease Control and Prevention (CDC).

    Dear all,

    We have recently realised that the CDC and WHO differ in recommendations regarding order of PPE removal.

    Currently we teach the order as per CDC (and as adopted by NHMRC guidelines) but are curious as to why the recommendation is to remove gloves, then goggles and then gown, where as WHO recommends gloves and gown first (likely being the two dirtiest items) and then goggles.

    Two varying posters are attached.

    Any thoughts would be appreciated.

    Dear members,

    I know that air sampling in a new building is a contentious issue but we are currently undergoing it as part of the commissioning process for new operating theatres and one of the difficulties I face is people asking for the acceptable limit of certain organisms. Aside from fungal organisms I have been unable to find any references to guide me on specific organisms counts.

    I would like to hear people’s views on the isolation of skin or environmental flora when doing this sampling – how many CFU would be acceptable per air sample ?

    No questions is a silly question. To not ask one is the silly thing! 🙂

    In my experience, the biggest problem with setting appropriate risk mitigation strategies for rehab units is not when they are standalone facilities, but when they are part of an acute facility. This is where the grey areas arise. If you have a rehab unit that shares staff and resources with acute care areas, this is where the specific policies for standalone rehab facilities may not always work best.

    It also depends on patient factors in the rehab facility. Does the facility manage patients with complex issues such as large open wounds, central lines, etc, or does the rehab facility not accept this type of patient until these things are considered controlled? This will also make a difference in risk management strategies to reduce the risk of infection transmission to these patients.

    Just something to keep in mind when discussing rehab facilities, I think. Like residential care facilities, the line between infection risk to acute facility patients and rehab facility patients is slowly becoming more burred.

    Dear colleagues,
    This may seem like question from a complete novice. However I am bravely asking it. Do hospitals which are purely for rehabilitation have different infection control policy and procedures from acute hospitals? I know that there will be some differences, but what about managing of patients with MROs, aseptic technique, and other infection control related matters? Is there anybody willing to share policies/procedures for rehabilitation facilities ? Any advice is welcome.

    Hi all,

    I am very interested in input from list subscribers to the issue surrounding use of wipes containing 70% alcohol and 2% CHG to clean the tops of vials prior to injecting or drawing up. Most subscribers would be aware of the adverse events reported in relation to injection of alcoholic CHG into an epidural that has altered practices in relation to insertion and management of epidurals. This concern has resulted; it appears, in a real concern in relation to the use of wipes on any equipment used in the management of epidurals. Please refer to the attached editorial. As the bottles of solution used are presented in a sterile form (sealed sterile packaging) we have recommended that the tops of these vials do not need swabbing prior to use. There do appear, however, to other real concerns relating to the potential for adverse events relating to both epidural and nerve infusions and possible contamination with chlorhexidine residues.

    Can I ask if anyone would like to make comment on your approach to this concern?

    I flag for those who may not be aware the information provided from the TGA in July 2014 when the TGA investigated a number of reported cases of an unusual infection that were associated with propofol (Ralstonia spp.). As a result of this specific investigation the TGA issued a statement which included the following general information;

    “The exterior surfaces of injection vials are not intended to be sterile. Most protective lids do not guarantee sterility of the outer surface of a vial rubber stopper/aluminium crimp seal. This lid is intended to act as a shield for the rubber stopper and to keep dust and other physical contaminants away from it.
    Noting this, health professionals are reminded that proper aseptic technique must be strictly followed when administering intravenous injections to a patient. This includes wiping the outer surface of the rubber stopper and its injection site with a suitable disinfectant wipe/swab and allowing it to dry before inserting any device into the vial.

    I have done this in other organisations I have worked in, and to say it was a pain is an understatement! The only real way to ensure you have captured ALL staff is to have a process for requiring them to complete a survey, then following up based on the responses to the survey (eg organising serology, boosters + serology, etc). You really need a good process for this, and a means to ensure ALL staff are captured, and ensure those who don’t initially fill in the survey get hounded until they do. It is very resource intensive and requires great record keeping, so really needs to involve HR from the beginning (to ensure all staff are included).

    If you are lucky enough to have a dedicated database which is linked to your HR database for this, it makes it easier, but still resource intensive to get it to the stage of only requiring new staff to be entered.

    I have to comment that this is a ‘borderline’ infection control activity, and can take valuable resources away from your infection control program for a significant period of time. It is best to be assertive with your facility executive managers and have them acknowledge the amount of time and resources this will require, and what impact it could have on infection control activities.

    I am awaiting a new staff health database here before I undertake this, and will be seeking a lot of data entry assistance. Good luck with however you seek to do this.

    Hi all,

    I am trying to get an understanding how other organisations are capturing data to be compliant with National Standard 3.6.1:

    – demonstrating maintenance of vaccination status of existing workforce employees
    – identifying subsequent additional vaccination requirements for relevant members of the workforce

    It is not a problem to get this data on new employees but I’m finding it difficult to find a good way to get the same level of info on existing staff. Any ideas would be very very welcome 🙂

    We no longer use ‘neutropenic precautions’ for any patient with neutropenia. The rationale for this is that they are more at risk from infection with commensals and their own flora than from others. We do place them in single rooms, and we do advise visitors who have infections not to visit, but no actual extra PPE (apart from what is needed for Standard Precautions of for cytotoxic therapy). We also try to roster staff who care for neutropenic patients to not also care for patients with overt infections during the same shift, where possible.

    We have not seen a rise in neutropenic fever or associated infections in this patient group since implementing this strategy.

    The question has been raised. Is it best practice to reverse barrier nurse febrile neutropenic patients or not?
    Staff have suggested that maybe patients are not getting the best care by being isolated as the patients feel neglected and shut off, our Oncologists feels that once antibiotics are commenced that there is no need to isolate, “standard precautions” is all that’s needed.
    I’m interested, what is the current practice out there?
