    Hi Matt
We use a procedure trolley for dressings in our residential aged care facilities. The trolleys are stored in the treatment room, cleaned and set up for a procedure as needed. The registered nurse usually does the procedures in the Drs room.
We completed our accreditation earlier this year with no issues highlighted regarding this practice.
    Kind regards

    Hi Matt,
I find this quite odd. We use procedure trolleys with sharps containers and ABHR attached in our residential aged care facilities. These procedure trolleys are stored in utility rooms when not in use. They are not stored in the resident’s rooms. This has never been raised at accreditation as being an issue. Indeed I would have concerns if the facility did not use procedure trolleys.

    Kind regards.

    Hi All,
It has been suggested to me that dressing trolleys should not be used in aged care facilities as this reduces the homely nature of the facility and can put the facility at risk in relation to their accreditation. Instead the facility wants to use the bedside table or bed to lay out the sterile field. I have a number of concerns about this, both from an OH&S and infection prevention point of view. Has anyone else come across this argument before?
    Cheers Matt

    Hi
We are auditing both the operating theatre and the recovery room. Two years ago we commenced a HH project in the OR and PACU to address poor HH compliance observed in the department. The implementation of our HH project was published this year in Healthcare Infection, March 2012: 17: 33-37.
We have re defined the patient environment in the operating theatre to suit high care areas such as the patient in ICU. The HHA observation & code classification tool has been updated to reflect this. Posters with the 5 moments of HH and hand hygiene product have been put strategically around the operating theatre complex. An education program and regular HH auditing of the unit began and low compliance rates have improved.
Theatre staff have Hand Hygiene as their portfolio and are now trained to do HH auditing. They submit data to the quality department monthly on HH compliance rates in categories of staff and HH moments in the OR and PACU. This project has also improved issues such as incorrect use of gloves and cleaning equipment between each patient.
We identified the departments with low HH compliance, provided education to visiting staff such as perfusionists, medical students, sales reps. The project has continued to evolve and some small changes have been made along the way. The anaesthetists have recently commenced a project to improve awareness of the 5 moments of hand hygiene and correct use of gloves.
I would be happy to share information to implement HH strategies in your Operating Theatres and PACU
    Kind regards

    Hi Joe,

We also do recovery, but also when HCWs come out of the operating theatre into the main complex and remove gloves (ie. wardspersons after helping position patients), then we see if they practice moments 3 or 4.

With regards education, we have posters explaining what the 5 moments are and we incorporate the moments into our mandatory hand hygiene practical competencies which everyone has to do every year.

Auditing is the same as everywhere else, same forms, same principle.
Barriers – none, except we don’t go into the actual operating room, same as with Bronwyn.

As for engagement with medical staff, we tell them that we are auditing everyone for hand hygiene compliance and we tell them that they are included. With all staff, we let them know that we capture data for all personnel.
As I work in theatres as well as the rest of the hospital, they all know who I am and why I’m holding a clipboard! One of our regular CMOs actually comes to tell me how often he’s washed/cleaned his hands so far every day!

Improvements have been that we have now got more standard auditors, and that all the doctors/patients/visitors (the persons who are the hardest to get to comply) and regular hospital staff, all use the new products. This is because a.) they like putting their hands underneath and having it do it for them! and b.) most people like the new products.

    Hope that helps!
    Helen.

    Hi

Is anyone doing the National Hand Hygiene 5 Moments audit in an operating theatre? If you are, I would be interested in finding out about your education program, process of auditing, barriers, engagement with medical staff and the improvements you have made.

Our operating theatre is very keen to be involved in participating in the audit

    Thanks
