    ACIPC members and particularly those previous ICA NSW Inc members will be saddened to learn of the passing of Prof. Yvonne Cossart OA on 16/12/14. Prof. Cossart was the initial Patron of ICA NSW and subsequently AICA. Based at the University of Sydney, she was a well-respected contributor to the Australian and international microbiology, infectious diseases and infection control communities.

    Perhaps most memorable were Prof. Cossart’s down to earth presentations provided to Sydney Hospital infection control participants from the late 1980s onwards. Yvonne was incredibly approachable, knowledgeable and innovative. She inspired and mentored several higher degree recipients and early infection control professionals alike. On behalf of many of those original NSW ICPs the majority of whom are now retired, I would like to extend our respect and sympathies to Yvonne’s family and professional colleagues at this sad time. RIP Prof. Cossart, Australian infection prevention is better for having known you and perhaps ACIPC Executive may wish to consider an ongoing tribute to you.

    Hello Marg. I agree with you. Needlestick injury is the big risk with using the insulin pens as they have to be recapped after use and the needles manually removed. Hence, manipulation leads to increased risk. We also use the QLD CHRISP advisory regarding not using these pens when there is an RN who is able to draw up insulin in a needle and syringe. At times, in the out of hospital setting, there is no other choice in this case. We document a risk assessment and training of staff to ensure they are aware of the risks and the safest possible way of management of the devices. There are some devices on the market to assist with removal of the needle however they are not supported by companies concerned to state they are a safety device

    Staff needlestick injuries from the use of pen needles has been an issue for some time. CHRISP produced an advisory of the use of pen needles some time ago.

    There is now at least one safety pen needle on the market in Australia, which are suitable for staff to use with most types of pen delivery devices. I can provide more details if you contact me off-list.

    Hi All

    Our physician wants pharmacy to dispense a prefilled insulin pen to the patient and nursing staff use this to administer the required dose. This pen would then be used by both the nursing staff & patient throughout the patients admission.

    I am wondering about the safety of using insulin pens as we have had a number of needlestick injuries from them in the past. If you do use then do you have procedures in place to prevent NSI that you would be happy to share

    From my experience, many facilities I have been to actually turn off the hand dryers at night because they are too noisy. The question has to then be asked ….how do they dry their hands? On their uniforms?

    Has anybody had any experience with installation of hand dryers (warm blowing air) in clinical areas?
    We are building a new facility, the question has been raised can we install hand dryers instead of paper towel in clinical areas at the hand washing sinks?

    Does anyone have any thoughts on this?

    Thanks Craig,

    Yes, we have been working on an Antimicrobial Prescribing Survey Tool for the past two years, with over 100 hospitals involved in a second nation wide pilot audit last year. We are working in collaboration with the ACSQHC, so the tool should meet accreditation standard 3 for auditing and possibly also used for bench-marking purposes. Currently an online database for easier data entry and analysis is being developed and we will be intending the tool be useful for all public hospitals (regardless of size), private hospitals, paediatric hospitals and aged care facilities. We will be advertising access to the tool and participation in further surveys later in the year once the database has been built and tested.

    In HONG KONG, we are also preparing for hospital accreditation program by following the AUS standards.

    As regards the disinfection of dressing trolleys practices, the auditors do not allow us using alcohol solution to disinfect it. However, it seems relevant requirement is not available in the accreditation manual. Hence, we would like to know the prevailing practices for disinfection of dressing trolley in Aus.

    Hi Matt
    We use a procedure trolley for dressings in our residential aged care facilities. The trolleys are stored in the treatment room, cleaned and set up for a procedure as needed. The registered nurse usually does the procedures in the Drs room.
    We completed our accreditation earlier this year with no issues highlighted regarding this practice.

    Hi Matt,
    I find this quite odd. We use procedure trolleys with sharps containers and ABHR attached in our residential aged care facilities. These procedure trolleys are stored in utility rooms when not in use. They are not stored in the resident’s rooms. This has never been raised at accreditation as being an issue. Indeed I would have concerns if the facility did not use procedure trolleys.

    Hi All,
    It has been suggested to me that dressing trolleys should not be used in aged care facilities as this reduces the homely nature of the facility and can put the facility at risk in relation to their accreditation. Instead the facility wants to use the bedside table or bed to lay out the sterile field. I have a number of concerns about this, both from an OH&S and infection prevention point of view. Has anyone else come across this argument before?

    Michelle, the process you have outlined is exactly what should be happening. It is so common to see overstocked dressing trolleys being wheeled from room to room in a RACF and of course, this is an infection risk. For some reason many RACFs seem to think there is a different standard required to any other health facility. Some of the reason for that is the issue highlighted by Matt.trying to maintain a homelike environment. The line between best infection control practice is being blurred.sometimes by people who dont have the IC focus we have. It can be most frustrating.

    Hi all
    I work in an aged care facility
    The issue I used to have with the dressing trolleys was staff kept stock on them all the time especially in draws despite education and constant removal of these items and , thus not cleaned between usage.
    I overcame this issue by getting smaller trolleys X1 per unit with no draws
    The trolley is kept in clinical room when not in use
    And staff clean the trolley prior and post use with neutral detergent
    (another issue as some staff wanted etoh for cleaning)
    It has not been an issue for accreditation when asked about these trolleys staff know what the correct procedure is.

    Hi All,
    It has been suggested to me that dressing trolleys should not be used in aged care facilities as this reduces the homely nature of the facility and can put the facility at risk in relation to their accreditation. Instead the facility wants to use the bedside table or bed to lay out the sterile field. I have a number of concerns about this, both from an OH&S and infection prevention point of view. Has anyone else come across this argument before?

    Another poor practice I have seen creeping in to a number of facilities other than the ABHR and glovesis the wearing of two pairs of gloves and peeling off the outside layer of gloves after performing a procedure and before going to the next personand also when caring in an isolation room!!!again totally inappropriate.

    I understand that this practice is being taught by some TAFE colleges and training organisations when training Cert ll and lll care givers. It amazes me that someone would actually teach this?

    I have seen this practice and have actively discouraged it continuing, it is often related to users wearing gloves inappropriately so looking at glove use may also assist with resolving the problem, It is specifically discouraged in the national infection control guidelines and also by Hand Hygiene Australia

    Marlize. Eltra by Ecolab is a laundry powder which is validated to meet the chemical disinfection requirements of ASNZ4146:2000. This product is suitable for top loading domestic type machines and may also be used in commercial machines (although manually dispesensed)
