    A clear decision needs to be made about whose responsibility it is to replace empty alcohol-based handrub bottles. Workplace agreements or job descriptions may need to be changed to accommodate prompt replacement of these bottles. Alcohol-based handrub should never be decanted from one bottle into another as this can cause contamination.

    Good afternoon,

    I have tried to look for evidence or a statement on the practice of decanting ABHR into smaller used bottles. I know that this is not best practice but would like to reference this please.
    I understand that some facilities resorted to this when ABHR supply was difficult to obtain, and I wonder if anyone else has had this dilemma and how you managed it?

    Hi Pam,

    Great question. See passage from the NHMRC Aust Guidelines below:

    Adapted from CDC Guideline for Isolation Precautions [215].

    Removal of PPE should be done at the doorway (just prior to leaving patients room) or immediately outside patient room.

    Surgical or particulate masks should be removed outside of the patient room or >1 metre from symptomatic patients [87].

    To remove a P2 respirator, remove gloves and gown, perform hand hygiene and step outside the room or into an anteroom. Remove protective eyewear or faceshield before removing and disposing of the respirator in a closed container and performing hand hygiene again.

    At a minimum, hand hygiene should be performed after the removal of gloves as well as after the removal of any other individual item of contaminated PPE.

    Sequencing may differ across healthcare settings and any variations to sequencing should be based on a risk assessment relevant to the setting and task being undertaken. Note that for surgical procedures and dentistry, the sequence for putting on PPE differs. In these situations, masks and protective eyewear are applied first prior to hand preparation. Gown and gloves are then put on (see Section 3.5.3).

    Sequencing may also be different in unique circumstances such as in an Ebola outbreak. When an outbreak is detected, advice from the healthcare facilities Infection Control Team should be followed.

    We have been discussing the same as we consider zones/airflow etc in our RACFs, I like the idea of the virtual anteroom outside the room and I have been working with our maintenance team to ensure quality tape on hand to identify these areas particularly to doff mask and eye protection outside the room.

    Good afternoon everyone,

    As more patients with COVID are admitted we are required to locate them in rooms that have no built anteroom.
    We are creating a virtual anterooms (Orange Zones) outside the patients room (Red Zone).

    We are having discussions as to whether doffing should occur;

    * outside the patient room (with the door closed behind) potential of contaminating environment
    * inside the patient room, extend the orange zone to inside potential for staff scrubs to be contaminated

    I have spent substantial time reviewing the literature for guidelines or information on the risk of each of these options but havent found anything definitive.

    I would really appreciate any advice.

    Hi Donna,

    Thank you for your reply. Not sure whether I am being too literal but the below link to the current Victorian Guideline for COVID-19 Publicly funded Vaccination Clinics states that temperature checking twice daily is required (p38). I would appreciate your further clarification on this.

    Hi Elizabeth,

    I have contacted the Victorian Department of Health Immunisation team re your query and received the following advice:

    Manual temperature checks of a vaccine fridge should be done at the start and end of every business day. There is no expectation that vaccine fridges are manually checked 7 days per week. In Victoria all government vaccine provider account holders must have a data logger in every purpose built vaccine fridge. The logger is routinely downloaded weekly or as soon as possible after a temperature excursion outside 2-8 degrees Celsius is detected to check the time frame for a potential CCB (longer than 15 minutes outside 2-8 degrees Celsius). The interval on the logger should be set at 5 minutely readings, as per the recommendations in the National Vaccine Storage Guidelines Strive for 5. All vaccine equipment should be serviced annually and a self-audit should also be undertaken at this time (https://www.health.gov.au/resources/publications/national-vaccine-storage-guidelines-strive-for-5-appendix-2-vaccine-storage-self-audit).

    Dear IC colleagues,

    The National Vaccine Storage Strive for 5 guidelines require twice daily monitoring before the refrigerator is used for the first time and at the end of each day. I am currently working where there is a COVID vaccination clinic that operates Monday to Friday. The Victorian Guidelines for this Clinic are different to the National Storage of vaccines guidelines and require temperature monitoring, twice daily, everyday Monday Sunday. This means that someone has to attend the clinic for temp checking over the weekend. Im just wondering if others can feedback how they are managing this requirement in real life.

    The below link to the guide to the conventional use of PPE directs that for short duration episodes of care for SCOVID/COVID patients a surgical mask may be worn however the attached would indicate that for all care of SCOVID/COVID residents an N95/P2 respirator must be worn.

    I believe that the question was in reference to the care of all residents (not SCOVID/COVID) however, and I would agree with Kelly that we are wearing surgical masks and eye protection.

    Not a simple world we are living in!

    The below link to the guide to the conventional use of PPE directs that for short duration episodes of care for SCOVID/COVID patients a surgical mask may be worn however the attached would indicate that for all care of SCOVID/COVID residents an N95/P2 respirator must be worn.

    I believe that the question was in reference to the care of all residents (not SCOVID/COVID) however, and I would agree with Kelly that we are wearing surgical masks and eye protection.

    Not a simple world we are living in!

    Hi Lucas

    Totally agree with Karen. You must also take into consideration that it can take up to 10 days for symptoms to appear after last contact. That is why we test at day 11 with 72 hours for results to come back. Hence 14 days isolation.
    Tough times for you guys at the moment but if you follow the guidelines it will get under control much quicker.

    Just curious what your thoughts and current policies are regarding new admissions to aged care facilities and quarantine.

    At present in our facility we are quarantining new admissions from the community or hospital for 14 days. This is following a recommendation from our local PHN.

    Are others following a similar policy? What are your thoughts on perhaps COVID testing new admissions and ceasing quarantine on return of negative testing?

    There is no requirement to COVID test all admission from the community or hospital just contact and droplet precautions for isolation 14 days.

    Hi Lucas

    Totally agree with Karen. You must also take into consideration that it can take up to 10 days for symptoms to appear after last contact. That is why we test at day 11 with 72 hours for results to come back. Hence 14 days isolation.
    Tough times for you guys at the moment but if you follow the guidelines it will get under control much quicker.

    Good evening all
    Just curious what your thoughts and current policies are regarding new admissions to aged care facilities and quarantine.

    At present in our facility we are quarantining new admissions from the community or hospital for 14 days. This is following a recommendation from our local PHN.

    Are others following a similar policy? What are your thoughts on perhaps COVID testing new admissions and ceasing quarantine on return of negative testing?
