    I know there is always a lot of debate of ‘Fit Testing’ for N95 Masks.

    Certainly happy to provide as much (or as little) resource.

    https://europepmc.org/article/med/20658919 : Link to large scale quantitative respirator fit testing program for HCW (SA Health, Prof Dino Pisaniello circa 2010). SA Health have had a fit test for many years now. Only Healthcare network in Australia to do so.

    * Extract of Order 43 Legislation (NSW) for different industry, but highlight new law for Quantitative Fit Testing introduced over past 18 months or.
    * Focus on Health of Frontline Staff
    * New OSHA Guidelines for 2.5 minute Fit Testing (released in Sept 2019)
    * Fit Testing for healthcare Professionals
    * Example of test report (Santa)

    Please don’t confuse the term Fit Testings vs Fit Checking – Quantitative Fit Testing is exactly that, a way of quantitatively measure the efficiency of the seal of the mask on the wearers face, unable to answer questions re stock.

    Hi all

    We are interested in other states/jurisdictions/facilities approaches to fit-testing, either qualitative or quantitative.

    We had a qualitative fit-testing program within our hospital previously and we made the decision to disband this earlier in the year for a number of reasons.

    We are now getting significant pressure from a particular clinical group to re-establish fit-testing again.

    We had a number of issues with the fit-testing program previously and our rationale for not re-establishing fit-testing currently include the following:

    * Fit-testing has not been proven to provide safer P2 (N95) mask use compared with fit-checking alone
    * Despite the presence of the Australian Standard, fit-testing is not mandatory and ‘fit-checking is accepted to be the minimum standard’
    * There are a number of practical difficulties to be considered with the re-introduction of a fit-testing program;
    * If fit-testing is offered to one craft group, all healthcare workers that require a P2 (N95) mask within their clinical role will expect a similar approach. Equitable approach would need to be considered for all.
    * Some individuals will not have a successful ‘fit-test’ e.g. do not taste the fit-testing solution, do not have a successful fit-test with any of the available masks. Does this equate to that healthcare worker being unsafe with an appropriately fit-checked P2 (N95) mask?
    * Resources required for fit-testing program is not insignificant i.e. human resources and PPE required, with consideration for annual testing thereafter
    * What to do for individuals that have not been fit-tested as yet but they are required to work with the requirement of a P2 (N95) mask?
    * What to do if individuals’ fit-tested mask is not available at the point of care, particularly in the setting of the current PPE stock issues?

    We would be really keen to understand others’ thoughts and experiences and approaches.

    Dont confuse the terms Fit Check with Fit Test they are different processes and the Quantitative Fit Test method is the only way of getting non subjective results.

    Hi Meredith

    We do not use any special equipment. We do undertake fit testing with new staff and it is a yearly competency for our ED staff. We talk them through the whole process of putting it on, blowing in and out, checking the seal for air leakage and removing safely. We advise on the correct size and discuss issues with beards etc.

    The 2019 Australian Guidelines p. 116 recommends fit testing as follows:

    * on commencement of employment for HCWs in clinical areas where there is a significant risk of exposure via airborne route
    * when there is significant change to the wearers facial characteristics (eg. weight loss or gain, etc)
    * annually thereafter (AS/NZS 1715:2009)

    There are two types of fit testing:

    * Qualitative – inexpensive equipment (approximately $400 per kit) requires hood and sprays, company rep can provide training to staff to perform fit testing, human resources required to perform fit testing on staff
    * Quantitative – expensive equipment, generally performed by mask company reps where a good seal cannot be obtained via a qualitative approach

    It is my understanding that all HCFs are recommended to undertake fit testing for staff, however my observation is that very few do. I am aware of two HCFs that participate in fit testing (1 x major public hospital and 1 private hospital).

    It would be interesting to know how many other HCFs participate in fit testing.

    Is anyone concerned that this (and other statements) ignores the requirement for fit testing of staff who are going to rely on airborne mask protection?

    Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

    Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

    I am like Emma, I have email all nursing staff, kitchen, housekeeping and wardies are to come to my office to be fit test as they have patient contact.
    In the email I sent them the directions for how to put the N95 on.
    I will be completing this yearly and added it to excel sheet.

    Hi Pam , just do my own locally using NHMRC guidelines for fit checking at orientation & then through inservicing throughout the year, weve also incorporated PPE into on line learning as well in IPAC module ,
    I know its not the ideal but we couldnt get the company here to do the fit test ( testing in the head box ect) and the cost at the time was also around $35/person

    Hi Helen,

    Our clinical worker health do a once off fit test and also competency assessment for donning and doffing of PPE.
    Fit testing is only repeated if the person has had significant weight gain/loss, facial surgery or major dental work.
    These tools might help you as they are similar to what our ICLN undertake.

    Hi Helen,

    We follow the SA Health Cleaning Standard and we clean the area with a single step product (detergent/disinfectant solution) and the same in disposable wipes.
    This just removes the guess work out of what to use.

    Morning Brain trust,

    I was looking for some advice in regards to infection control in theater.
    I was wondering in anyone can tell me how they clean the operating theater after MRSA, VRE, ESBL.
    Recently a staff member said that we are over doing the cleaning in regards to what I do and we only need to use soap and water.
    I give them 2 options of how to clean with the products, which I cant mention on here I believe.

    I was just wondering what is a good cleaning regime for all the codes?

    Any information would be helpful,
