    Dear Brains Trust,

    Our facility recently underwent accreditation and there was surprise expressed at our ongoing screening of patients for VRE, with verbal reports that a number of hospitals no longer routinely screen for VRE.

    I would like to know what people are doing in their facilities

    * Do you screen or not screen?
    * If you screen, what patient population is screened?
    * If you have stopped screening – Why?
    * If you have stopped screening what is the impact on VRE infection rates?
    * How are VRE positive patients managed?

    Hi Fiona,

    We screen all renal patients on a quarterly basis.

    We screen all patients who have been admitted for an overnight stay outside WA then we manage them on contact precautions in isolation until the results are received.

    VRE inpatients are managed in a single room with an ensuite on contact precautions.

    * Yes, we screen as per the following:
    * High risk units – ICU – On admission and weekly thereafter
    * Ambulatory units – dialysis – Prior to admission and every 3 months thereafter
    * People who are identified as a VRE contact
    * Stool, rectal or perianal swabs are considered a sensitive method for detection of VRE

    * Do you screen or not screen?
    * Yes, we screen as per the Queensland Health guidelines (link as posted by Meredith
    * If you screen, what patient population is screened?
    * ICU – weekly, all cardiothoracic patients pre-op, direct transfers from other hospitals, those with an overnight hospital stay in the past six weeks, or with a past history of VRE.
    * If you have stopped screening – Why?
    * Not applicable
    * If you have stopped screening what is the impact on VRE infection rates?
    * Not applicable
    * How are VRE positive patients managed?
    * As per QHealth, with single room and contact precautions in wards. We also have specific precautions in procedural areas simply because we do not yet routinely clean with disinfectants.

    HI Fiona,

    Screening for haemodialysis patients

    KHA-CARI draft haemodialysis infection
    control guidelines (2017) were indicating that screening for VRE in haemodialysis
    patients was no longer required.


    However still waiting for these haemodialysis
    infection control guidelines to be finalised and reposted on their website.

    We no longer routinely screen for VRE
    in our dialysis clinics. We manage all patients with strict standard precautions
    and strict aseptic technique.


    No affect on our VRE infection rates,
    which were previously rare anyway.
