Requested Explantable items and Human Tissue not required for pathology

Good Afternoon everyone,
I am looking for some help with reviewing our policy in operating suite/CSSD on Requested Explantable items and Human Tissue not required for pathology.
We currently have a Policy and Patient Disclaimer, but after reviewing this in line with HICMR, I think we are missing part of the process in our documentation.
Has anyone got something they are willing to share, for us to review?
