Hi All,
We are a facility that still uses paper based medical records our concern is that these may become widely contaminated and pose a risk to the medical records staff when they are reviewed / coded.
Whilst we only have minimal patient numbers we are able to manage the records outside the patient room, however in the event of a full ward of patients we are concerned contamination is likely to occur.
Are you able to share with me how your facility is managing paper records from COVID positive wards?

Dear Fiona – for GPs where a pt still has a paper /script to hand over I
ask staff to open a plastic pocket (there are millions stashed away in
offices and office supplies all over this country) and pt pops it in – no
touchies. I know it doesn’t go through the fax or scanner but we have to be
creative so1.If not urgent then leave in plastic pocket for some days until you
have nothing else to do!

My hospital is the same.
All of our patient records are paper based. I encourage all patient files
to stay in the discharge lounge and obs taken in patient rooms, then hand
hygiene prior to touching the file.However, many hands do touch the file along the patient pathway through
surgery. It is hard to track. I would also welcome any input from the

We have paper records that then get scanned at this stage.
My view has always been that we should remind staff that it is not what they touch that is important, but what they then do with their hands. So, wash your hands people!
We know there is MRSA and VRE on lots of surfaces within healthcare environments. But the only way that will cause problems is if we let them by not performing hand hygiene.

Michael I understand but then staff start to spend time cleaning the scanner then it never stops. I am assuming staff are doing hands anyway but contact spread via machines? Surely we can we try to reduce this – machines getting wiped down with some probably damaging chemicals atm by untrained staff in admin who dont have the advantage of our knowledge.

Hi Fiona,
We have decided that we are going to make up a whole new chart for the patient for progress notes,etc.
When patient gets discharge we are going to fax down to medical records and they will print and place in medical chart.
We will be number all of our pages.

Hi All,
We are a facility that still uses paper based medical records our concern is that these may become widely contaminated and pose a risk to the medical records staff when they are reviewed / coded.
Whilst we only have minimal patient numbers we are able to manage the records outside the patient room, however in the event of a full ward of patients we are concerned contamination is likely to occur.
Are you able to share with me how your facility is managing paper records from COVID positive wards?

We have decided that we are going to make up a whole new chart for the patient for progress notes,etc.
When patient gets discharge we are going to fax down to medical records and they will print and place in medical chart.
We will be number all of our pages.
