    Hi All

    I am fairly new to therole as acentralisedIPC lead.     This was not something that I have implemented, but was there in place. When I started our HH audit, my audittemplate was based on 4 moments. I find this quite misleading for the staff given there are now2 different instructionsto follow. I was wondering if other organisation follow 4 or 5 moments. If 5,how do you make sure this is followed correctly by staff.

    Hi Tenzin,
    At our health service our aged care facilities follow the 5 Moments and we teach the same messaging as non-aged care facilities (simple message is clean your hands on the way out of patient zone even if you only touched the surroundings, not the patient). Shared resident areas are managed as per a shared gym or something similar in a rehabilitation area. If you are referring more to issues with ABHR placement and not having the product in single resident rooms due to potential risks we manage this as per any other setting and assess for potential risks, restrict access where needed but place ABHR on all shared equipment, trolleys etc. to optimise access when a health care worker is around.
    We have many staff that work across Aged Care, Multipurpose Health Services and acute settings if we changed our messaging it would be super confusing for staff.

    Hi Tenzin,

    I understand your query regarding 4 vs 5 moments of Hand Hygiene. As all our (Anglicare Southern Qld) resources refer to the 5 moments, I have decided to stay with this even though moment 2 is not relevant to the majority of our staff, (the AINs, HCWs, PCWs.) However removing it would not be suitable for the RNs and EENs who are performing procedures. We have introduced a ‘procedure tray’ that has a sharps container, a kidney dish and hand hygiene on it to take to the bedside/resident when procedures are performed and Hand Hygiene is required, this means we don’t have to place ABHR on the beds.

    During my education and workshops I address the five, or four moments of Hand Hygiene depending on my audience and this seems to suit our organisation’s approach to transferring the acute focus of infection prevention to the residential environment. Always a challenge for us as IPCs in Aged Care.

    In NSW, we follow the NHHI guidelines.
    Some principles that might help you are:

    1. The 5 Moments.
    2. Moment 1 – before touching a patient.
    3. Moment 2 – before a procedure.
    4. Moment 3 – after a procedure or body fluid exposure risk.
    5. Moment 4 – after touching a patient.
    6. Moment 5 – after touching a patient’s surroundings.

    Also, it is not hard to incorporate the changes, simply make up some posters or use the ones supplied in the NHHI guideline booklet.

    1. Touching a patient in any way Shaking hands, assisting a patient to move, allied health interventions, touching any medical device connected to the patient
    (for example, intravenous line pump, urinary catheter)
    2. Any personal care activities Bathing, dressing, brushing hair, putting on personal aids such as glasses Any non-invasive observations Taking a pulse, blood pressure, oxygen saturation, temperature, chest auscultation, abdominal palpation, applying ECG electrodes, cardiotocography
    3. Any non-invasive treatment Applying an oxygen mask or nasal cannula, fitting slings/braces, application of incontinence aids (including condom drainage)Preparation and administration of oral medications Oral medications, nebulised medications Oral care and feeding.
    4. Feeding a patient, brushing teeth or dentures

    I hope this helps you I do know that SA health guidelines are stringent and surprised your facility is not following these.

    I agree 100% re 4 moments in aged care – a RACF resident does not neatly sit within a boundary such as a room or patient area as they would in a hospital setting

    There are many shared spaces, such as dining rooms, activity areas, cafes etc

    At my organisation we combine moments 4 and 5 as you describe

    Based on the Canadian Ontario Health ‘Just clean your hands’ (long term care) program https://www.publichealthontario.ca/en/health-topics/infection-prevention-control/hand-hygiene/jcyh-ltch or https://www.youtube.com/watch?vWQKu7V2sy64 , they have some amazing resources for an aged care setting

    SA Health adapted the tool we developed at my organisation based on the Ontario model for use by providers who do not contribute to the NHHI
    This tool more accurately reflects an aged care setting
    Instead of identifying Moments 1, 2, 3 etc we assess against opportunities observed and whether HH was undertaken

    Initially we developed posters and education focussing on the 4 moments, but over time most signage reflects the standard 5 moments for HH.
    I don’t really get hung up on 4 / 5 moments, I talk to the opportunities which is observing HH being undertaken prior to and after any care / person / environment contact

    At the end of the day our focus is on improving hand hygiene practice ensuring improved resident outcomes

    Thank you everyone for taking time to response with best practice. I really appreciate each one of you for your response and sharing your best practice and they are valuable. Thank you in particular to Robyn for your reassurance and sharing the observation tool which I was not aware of and this will be a great tool for me to apply during my next HH observational audit.     Initially we developed posters and education focussing on the 4 moments, but over time most signage reflects the standard 5 moments for HH.
    I dont really get hung up on 4 / 5 moments, I talk to the opportunities which is observing HH being undertaken prior to and after any care / person / environment contact
    IA bit hard to explain in an email sorry

    At the end of the day our focus is on improving hand hygiene practice ensuring improved resident outcomes

    I am fairly new to therole as acentralisedIPC lead. Prior to taking on the role, in the recent timemy organisation had adopted 4 moments of hand hygiene instead of 5 moments and therationale behind this is 5 momentsapparently doesntsupport aged care setting.     This was not something that I have implemented, but was there in place. When I started our HH audit, my audittemplate was based on 4 moments. However,recent SA health onlineinfection course that all aged care workersrequired to complete still has 5 moments. I find this quite misleading for the staff given there are now2 different instructionsto follow. I was wondering if other organisation follow 4 or 5 moments. If 5,how do you make sure this is followed correctly by staff.

    Any procedure can be emptying the IDC or diaper removal.Cleaning their dentures or brushing of the teeth.
