I know this is a tangent thread to the original question – however
The laundry service described is within the scope of AS/NZS4146:2000, because it meets the definitions of either Category a commercial (nursing homes and hospitals: products serviced include personal clothing), Category c (hospitals-private, public and extended care facilities or general care facilities: products serviced include patient apparel) or Category e on premises (nursing homes and private hospitals providing a service for own internal use).
As Paul said the thermal and or chemical disinfection requirements for soiled linen (section 3.5) are quite clear, and I believe, should apply. There may be a ambiguity if use of the washing machine is restricted to only personal use by the individuals in rehab. However once staff and other purposes become involved (and inevitably they do) then the purpose and the setting will bring the service within the scope of the standard.
commercial relationship – The definitions use agency type and products serviced rather than payment.
Coin operated laundries – included within the scope of the standard.
Normalization of a home like environment – is a great philosophy and goal, however the infection risk matrix changes when many people (including people at risk of infection and people with infections/ colonization’s) are accommodated in a communal living environment. It must be recognized that this environment poses a higher risk of infection transmission than a residential home occupied by a small healthy family; and therefore enhanced infection prevention practices may be necessary.

I have included a couple of posters we have had for quite a number of years now. We do allow siblings to visit & do not appear to have a problem. Only once did we have RSV come into the unit but that could have been from staff or other adult visitors (this baby did not have siblings).I believe babies in NICU have more bugs of concern that those in the community. Hand hygiene can never be underestimated and parents, children or staff who are sick should be educated not to come into NICU or SCN.

I would love some feedback regarding restrictions of visitors to Special care nurseries.
Who do you restrict? Other siblings?
I feel as long as Hand hygiene is performed and the visitor is well I personal feel that there is no real significant risk.
Some of our staff feel that in the case of twins, one goes home and one stays in the nursery then the well twin shouldn’t visit as the baby might pose a threat to the babies in Special care (perhaps from a whooping cough point of view).

I suppose the question has to be asked: what will they be washing in this washing machine? If it is their own personal clothes, why do you require a disinfecting additive? If it is not a commercial washing machine with automatically dispensed and monitored chemical (which I would think would be unsuitable for a rehabilitation program, unless they were all laundry workers or return to work programs….), and is not used for commercial washing (eg someone pays for the service provided) then it only needs to be similar to domestic standards, not commercial. That is my understanding, anyway. If the patients were using a Laundromat they would not need to use a disinfectant, and someone else’s clothes have been washed in the previous cycle.
Just my thoughts. If you are still wanting a commercial grade chemical disinfectant for laundries, I would be contacting a commercial laundry for some suggestions.

If the patient is washing their own clothes then a disinfectant is not
required. I would imagine it is a domestic washing machine that you have
purchased and not a commercial washing machine. AS4146 is quite clear
about chemical disinfection. Most commercial washing machines achieve
disinfection through thermal methods of above 65degrees. I would say if
you require the items to be disinfected I would use a commercial company
to do it as you would have to conducting monitoring and testing.A cheap and efficient way to disinfect clothing is to use a tumble
drier, it can reduce bacteria in clothing by a 100fold.Paul Smollen

Subject: Re: Laundry detergentIt’s my understanding that once the wash and rinse cycle is completed there is no need for the machine to have either a wipe out or empty wash cycle before the next user. Our rehab units utilise domestic washing machines and use the same parameters for washing their clothes as you would in the home. Most bio-load (but maybe not stains) is lost in the wash cycle as combination of sudsing effect and surfactant effect from the detergent combined with the movement of the water, then rinsed off and away in the rinse cycle [99% lost in the wash…] No need for disinfectant. Some of our clients have in the past done an empty cycle before using the machine for their own clothes. But, why would you if you’re willing to put items into a commercial wash house – washing machine. Might I also say that the water to washing machines is not through a mixer and so the machine (if modern) will mix the water to the parameters of the wash cycle on the individual machine. Another thing, rehab units are about normalisation, there philosophy is in my experience ‘if you don’t do in your normal environment (the home) don’t bring it into the rehab unit’ and we have this philosophy embedded policies for rehabilitation sites. Its the clients who wash their clothes but given that they have dubious personal hygiene and many have blood borne viruses, I am not happy to allow.
