Hi everyone,
I have request by builders to put jumbo toilet roll holders – that hold large round rolls- in ensuite rooms on a new ICU build. My thoughts are no it is an infection control risk but I thought I would ask my more experience learned colleagues in case you have done this?.

The only issue in my view would be from a discharge cleaning perspective. If you normally dispose of paper (non-wipeable) products after an MRO in the room, for example, this would potentially increase wastage.
Otherwise, I would agree it is not an infection control issue.

Not ICU, but we have them in our day hospital, can’t really see why it would be considered an IC risk, it is a toilet after all??

Hi Cate,
I am aware that Professor John Ferguson has raised concerns about quality of toilet paper e.g. cheap 1 ply, as used in hospitals, as a potential risk for VRE transmission.
Maybe you could contact him for some further evidence to support not installing jumbo rolls.
I find it intriguing that hospitals install jumbo 1 ply toilet rolls usually in very low wall mounted toilet roll dispensers which are very difficult for patients to use (find the end of the roll) hence contaminate the roll with dirty hands, plus the toilet paper usually ends up on the floor in a long strip which patients will pull up to use.
Surely single leaf toilet paper dispensers mounted at a height that a patient & staff can more easily access would have to be better but are deemed as ‘expensive’ & messy as the leaflets can fall on the floor as does the toilet roll paper but the single leaflets are much harder to pick up!
However single leaf dispensers are easier for patients to use especially if mounted at a correct height plus there is generally less wastage, the paper is better protected from contamination & no sharp edges from the paper cutting edge.
Have you asked the opinion of an occupational therapist? I have previously worked in a small geriatric hospital & it was a very interesting exercise trialling toilet paper – there were many OT issues & the single leaflet paper was definitely the easiest for the patient to use.
We are currently planning our fixtures & fittings for our redevelopment. Our redevelopment team have chosen jumbo toilet roll holders however the toilet paper is dispensed from the centre of the dispenser to reduce risk of contamination by patient hands & the roll of paper falling onto the ground. I will send you a photo.

Hi Cate,
I would suggest that you refer to the Standard Components provided as part of the resources within the Australasian Health Facility Guidelines.

On referring to the recommendations for ensuite bathrooms there is clear evidence that a small standard toilet roll holder is what is drawn in all ensuite Room Layout Sheets for single, double or 4 bed shared ensuite toilet facilities (described as ‘dispenser toilet paper’)
I think that you would do well to refer your design team to these standard components. I agree that the risk may generally be minor, but think in a broader risk management context that toilets and bathrooms should be regarded as being of higher risk for transmission of important pathogens and recognise that cleaning of complicated larger dispenser may be problematic and potentially ineffective sometimes.

What an interesting topic. It seems that the only issue raised is wipeable surfaces. Can I please take this opportunity to raise thinking, and likely debate about the adjunct method of disinfection – no waterless disinfection.
As many of you may realise there are now several different types of either hydrogen or xenon-based waterless disinfection system. These systems are now almost routine in US hospitals and are being adopted in the UK and other parts of the world as they become available.
There is good evidence for their effectiveness and contribution to reducing transmission risk and infection. They are NOT an alternative to but an ADJUNCT to cleaning. As a healthcare consumer my hope is that they soon become routine in Australian hospitals.
Happy to talk more about them to anyone who may be interested. I know that summary papers and other research papers have previously been shared here and I am also happy to share those again.

Hi everyone,
I have request by builders to put jumbo toilet roll holders – that hold large round rolls- in ensuite rooms on a new ICU build. My thoughts are no it is an infection control risk but I thought I would ask my more experience learned colleagues in case you have done this?.

Hi Cate,
I am under the impression that the toilet paper leaflet dispensers are more cost effective, available in single and multiple ply. These dispensers have no possibility of contamination and the dispenser is cleanable.Richard Bartolo
