Hi all,
Is anyone concerned that this (and other statements) ignores the requirement for fit testing of staff who are going to rely on airborne mask protection?
Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

Hello Everyone.
On the back of John Ferguson’s comment. How does one go about implementing fit test training that is, is there an accredited course or Trainer one can co-opt to educate staff? Do any of the mask companies provide this training. Would be interested in how other sites meet this compliance requirement as we are as small private hospital licensed 78 beds.

Just commentary and noting that this page John refers to was last updated in 2008
There has been further discussions since then re the cost implications of fit testing and scientific evidence to support the practice.
The Australian ICGs also recommend that when removing PPE that protective eyewear is the 2nd item to be removed this was subject to considerable debate during ebola preparedness and some jurisdictions now have different removal sequences to protect eyes.

The 2019 Australian Guidelines p. 116 recommends fit testing as follows:
* on commencement of employment for HCWs in clinical areas where there is a significant risk of exposure via airborne route
* when there is significant change to the wearer’s facial characteristics (eg. weight loss or gain, etc)
* annually thereafter (AS/NZS 1715:2009)
There are two types of fit testing:
* Qualitative – inexpensive equipment (approximately $400 per kit) requires hood and sprays, company rep can provide training to staff to perform fit testing, human resources required to perform fit testing on staff
* Quantitative – expensive equipment, generally performed by mask company reps where a good seal cannot be obtained via a qualitative approach
It is my understanding that all HCFs are recommended to undertake fit testing for staff, however my observation is that very few do. I am aware of two HCFs that participate in fit testing (1 x major public hospital and 1 private hospital).
It would be interesting to know how many other HCFs participate in fit testing.

Is anyone concerned that this (and other statements) ignores the requirement for fit testing of staff who are going to rely on airborne mask protection?
Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

If organisations are contemplating qualitative fit testing of staff members, it should be noted that there are also differences in results between using the sweet and bitter solutions in the 3M qualitative kit. My understanding is that you get better results with the bitter solution but not all people can taste it.
However, the key thing here is to ensure that you have different sizes of respirators available and also different shapes- a duckbill shaped/flat fold product does not suit all face shapes. Instead, people without a prominent bridge to their nose should be using a cupped version.

I think that we should be aware that the CDC actually has two options for PPE removal and our NHMRC only supplies a poster on Option 1.
With my past history as a scrub nurse, Option 2 makes way more sense to me than Option 1.

Just commentary and noting that this page John refers to was last updated in 2008
There has been further discussions since then re the cost implications of fit testing and scientific evidence to support the practice.
The Australian ICGs also recommend that when removing PPE that protective eyewear is the 2nd item to be removed this was subject to considerable debate during ebola preparedness and some jurisdictions now have different removal sequences to protect eyes.

On the back of John Fergusons comment. How does one go about implementing fit test training that is, is there an accredited course or Trainer one can co-opt to educate staff? Do any of the mask companies provide this training. Would be interested in how other sites meet this compliance requirement as we are as small private hospital licensed 78 beds.

Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

We do not use any special equipment. We do undertake fit testing with new staff and it is a yearly competency for our ED staff. We talk them through the whole process of putting it on, blowing in and out, checking the seal for air leakage and removing safely. We advise on the correct size and discuss issues with beards etc.

The 2019 Australian Guidelines p. 116 recommends fit testing as follows:
* on commencement of employment for HCWs in clinical areas where there is a significant risk of exposure via airborne route
* when there is significant change to the wearer’s facial characteristics (eg. weight loss or gain, etc)
* annually thereafter (AS/NZS 1715:2009)
There are two types of fit testing:
* Qualitative – inexpensive equipment (approximately $400 per kit) requires hood and sprays, company rep can provide training to staff to perform fit testing, human resources required to perform fit testing on staff
* Quantitative – expensive equipment, generally performed by mask company reps where a good seal cannot be obtained via a qualitative approach
It is my understanding that all HCFs are recommended to undertake fit testing for staff, however my observation is that very few do. I am aware of two HCFs that participate in fit testing (1 x major public hospital and 1 private hospital).
It would be interesting to know how many other HCFs participate in fit testing.

That sounds like fit checking not fit testing. Fit testing requires the use of the equipment previously mentioned and is a requirement of the respiratory protective equipment standards.

We do not use any special equipment. We do undertake fit testing with new staff and it is a yearly competency for our ED staff. We talk them through the whole process of putting it on, blowing in and out, checking the seal for air leakage and removing safely. We advise on the correct size and discuss issues with beards etc.

The 2019 Australian Guidelines p. 116 recommends fit testing as follows:
on commencement of employment for HCWs in clinical areas where there is a significant risk of exposure via airborne route
when there is significant change to the wearers facial characteristics (eg. weight loss or gain, etc)
annually thereafter (AS/NZS 1715:2009)
There are two types of fit testing:
Qualitative – inexpensive equipment (approximately $400 per kit) requires hood and sprays, company rep can provide training to staff to perform fit testing, human resources required to perform fit testing on staff
Quantitative – expensive equipment, generally performed by mask company reps where a good seal cannot be obtained via a qualitative approach
It is my understanding that all HCFs are recommended to undertake fit testing for staff, however my observation is that very few do. I am aware of two HCFs that participate in fit testing (1 x major public hospital and 1 private hospital).
It would be interesting to know how many other HCFs participate in fit testing.

Is anyone concerned that this (and other statements) ignores the requirement for fit testing of staff who are going to rely on airborne mask protection?
Btw, CEC NSW have developed nice combined Contact/Droplet signage for those with geographical signage space deficiency (GSSD). Kathy D am sure will share it.

Dont confuse the terms Fit Check with Fit Test they are different processes and the Quantitative Fit Test method is the only way of getting non subjective results.

Hi Phil,
I’m seeking some clarification in relation to the information sheet with
interim recommendations for the use of personal protective equipment (PPE)
during hospital care of people with Coronavirus Disease 2019 (COVID-19) you
posted below.Specifically the number of times staff are required to wash their hand when
removing PPE as per instruction below:*Perform hand hygiene before donning gown, gloves, eye protection
(goggles or face shield) and a P2/N95 respirator, which should be
fit-checked.
*After the consultation, remove gown and gloves, perform hand
hygiene, remove eye protection perform hand hygiene, remove P2/N95
respirator and perform hand hygiene. Do not touch the front of any item of
PPE during removal, perform hand hygiene at any point contamination may have
occurred.
*The room surfaces should be wiped clean with detergent/disinfectant
by a person wearing gloves, gown and surgical mask.
*The room should be left vacant with the door closed for at least 30
minutes after specimen collection (cleaning can be performed during this
time by a person wearing PPE).My understanding is that this requirement is specific to Ebola and is not
necessary for Coronavirus?I note it is not a requirement in the CDC guidelines – Interim Infection
