Dear Colleagues,
Just wondering if anyone has an annual hand hygiene award that is awarded to ward/department, and if so what are your criteria for selecting the winners?
We are about to award for the first time in our annual awards, but want the selection to be correct as it is not just about improvement but consistency of improvement, we don’t want to put staff off improving on hand hygiene compliance who have had good results but maybe have not been consistent!! Does that make sense???Anyway happy to hear your advise:)

Hi Jayne,
At the RHH we introduced a recognition award scheme some years ago. Most recent years we have awarded a ‘Certificate of Achievement’ to both the most improved unit across our organisation during the past 12 months as well as an award to the unit achieving the highest compliance overall during this time. We only do it annually in conjunction with World HH day, thus we have the greatest number of services to choose from. We always get our CEO or someone from senior executive to be involved and present the award in the main entry of the hospital. We also had them framed so that a unit could display this for visitors etc. to see to promote HH.
We have also commenced a recognition program for our auditors whereby at the end of the year whereby we present them with a certificate of achievement. As HHA state to maintain your registration as an approved auditor you need to collect at least 100 moments, we give these certificates to those auditors who meet or exceed that total . We actually state how many moments they collected. This is a really simple way of saying thank you to people. I have attached my own certificate as an example.
We have kept it pretty simple!

Hi Jayne,
At Sydney Children’s Hospital we have also given awards for hand hygiene – most improved ward, most consistent ward and ward with the highest percentage of compliance. They got a large box of chocolates and a certificate.
We have also given the auditor with the most audits done and the most consistent auditor recognition with a coffee voucher.
They all get their names mentioned in our hospital newsletter online.
Our Hand Hygiene committee decides on these awards at the end of each audit period.
We have also discussed putting up a notice board of recognition of these achievements in the entry foyer but have yet to act on this idea.Yes, it is time consuming and the money for these things needs to come from somewhere as well (despite it not being very much money). However, the rewards are positive reinforcement and have generated some friendly rivalry.

Hi Jayne,
We have a monthly report we do which includes the number of moments and compliance for each ward/area, we also include a doctor v nurse compliance graph as a bit of friendly rivalry – although the docs have never come close. Any ward/area that achieves over 85% compliance and has collected over the number of moments required for their area that month (number is agreed depending on ward occupancy and throughput) gets a laminated poster to display on their notice board. We have a monthly hand hygiene auditor meeting and give two movie tickets and a laminated “congrats” poster to the auditor with the most moments & compliance, we also give out a coffee voucher to the 2 next best auditors.Our Clinical Nurse Managers also have their ward/area hand hygiene compliance as part of their monthly KPIs.
We have found that doing the monthly in-house stats has kept the program in the focus of our staff and has helped embed the culture of complying with hand hygiene into the norm of the organisation.

Hi Jayne
Our Executive have an incentive awards program which does reward areas which started ahead of Round 3 2015. Our health service has all clinical inpatient areas audited and we have submitted over 4500 observations as a health service for Round 1.
Our criteria is that areas must have achieved the following to receive the award:
* 100% compliance with mandatory hand hygiene elearning for the last 3 consecutive months
* areas that exceed their respective national peer aggregate score in a round of auditing can choose $1k of patient equipment for their area – they can roll this over for 3 rounds in the year
* the areas that are the highest performing in results in their respective categories (we have subacute, acute and high risk acute) across the 3 rounds of auditing in that 12 month period are able to choose $5K piece of patient equipment for their area
* areas must do their own audits using local trained or GS auditors
* areas must display their results of each round in a consumer friendly format.To that end we have had trained Gold Standard auditors and we also run local auditor courses with the expectation of at least 3 per ward. My team coordinate the areas and do inter-reliability with the auditors as well as support the auditors and NUMs with the usual issues. We also assist in some areas to do their auditing where necessary. We have 2 hospitals and in Victoria a higher state target than the national target.
Not all areas achieve the required peer aggregate target results so awards have been small numbers to date but overall each site has achieved greater overall results than either the Victorian acute target or their respective peer aggregate results. This is the first round that has not been time critical or stressful getting it all done as the auditors started in Dec and do small frequent moments.
This has really helped to foster local ownership and accountability for their results. The NUMs also have a KPI of 10% improvement in this and employee immunisation (all not just flu vaccine) in their annual performance review. This has freed us up to focus our resources on employee immunisation as we now give approval for commencement for all new employees and provide the service.

I read the responses to your question with great interest. There are some good ideas. I would encourage you to be very very innovative in what you introduce. Make your system fun, make the reward something that is valued and perhaps look even to award criteria outside of healthcare so you can find a pattern or an idea that could be adapted. There are numerous fields where 100% compliance is the Gold Standard and I would even encourage you to not make awards if you get a consistent rate far short of 100% or you don’t get substantial sustained compliance, otherwise who knows if the results were by chance only and who would reward chance? I know there is fabulous Aussie research coming out about over estimation by human auditors compared to non-human observations.For examples of other industries look for food awards (5-hats etc), aviation safety or airline of the year, hotel of the year or even burger of the year. I hope that this idea makes sense and doesn’t infer all I do is travel and eat burgers (far from the truth). There are also ranking systems for universities and US hospitals which could give you some innovative ideas. If you would like to talk about this offline I am happy to help you. Otherwise good luck and hopefully you will proud enough of whatever you develop to share it here.

At Sydney Children’s Hospital we have also given awards for hand hygiene – most improved ward, most consistent ward and ward with the highest percentage of compliance. They got a large box of chocolates and a certificate.
We have also given the auditor with the most audits done and the most consistent auditor recognition with a coffee voucher.
They all get their names mentioned in our hospital newsletter online.
Our Hand Hygiene committee decides on these awards at the end of each audit period.
We have also discussed putting up a notice board of recognition of these achievements in the entry foyer but have yet to act on this idea.Yes, it is time consuming and the money for these things needs to come from somewhere as well (despite it not being very much money). However, the rewards are positive reinforcement and have generated some friendly rivalry.

Thank you, there have been some great responses, I agree the award has to be innovative and meaningful. Making the process fun along the way always makes embedding change or new initiative easier. I know HHA isn’t new but new ways of getting the message across is always difficult and giving incentives however big or small helps.
We plan to present the award at our annual quality awards dinner, a perpetual plaque that can be engraved and kept for the year. So we are trying to come up with criteria for judging the award that is fair to all but obviously to the department that are most consistent with their compliance.Kind regards
