Hi All,
For those of you that complete endoscopy water testing as per AS4187 Table 7.3…
I would like to know if anyone has decided to create a tolerance level for the Total Viable Count (TVC)? (I am not asking this for any of the specific organisms we test for, just TVC)
Table 7.3 requires TVC <10cfu per 100ml – if you don't achieve this and your level is over 10, do you:
* immediately close your automated endoscopy reprocessing (AER) machine and run a disinfection cycle / change filter / retest whilst keeping your machine out of operation until clear results are seen?
OR
* immediately run a disinfection cycle / change filter / retest whilst allowing your machine to continue to be used after this?
* If this is your option – do you have an agreed level of HCC that this applies to i.e. up to 100cfu per 100mL as an example?Essentially, I would like to know if anyone stipulates a range where they are happy to continue using the machine even though TVC may be over 10cfu?

Hi Kate,
The updated 2021 Infection Control Guidelines for Endoscopy – available through GENCA, now have a flow chart for AERs (on page 60).
I basically follow this, though you can end up in a never ending loop!. Noting it can take a while to fix biofilm related water contamination.
We have also discussed with the company, there may be other options to assist to remove biofilm etc.
