Dear team
I have a question are your staff using drawing up needles to prepare IV flushes and ABs from plastic vials or swabbing the vials with alcohol wipes then direct connecting the syringe to draw up?
I would like to know so I can teach best practice infection control to the students.Kind regards

Hi Leanne,
The NSW MoH Infection Prevention & Control Policy PD2017_013 section states;
Flip-top pharmaceutical vials are a dust cover and therefore all vials must be cleaned prior to access to maintain aseptic technique.
Injectable medication or solution must be taken from a vial or ampoule (or other similar container) using a sterile needle and syringe to withdraw the contents.
Before each entry into the multi-dose vial the top must be cleaned and injected with a new unused sterile needle and syringe, even if the vial is dedicated to a single patient.

Hi Leanne,
It is standard procedure to use a drawing up needle for saline and to swab vials and let them dry before piercing them. For flushes best practice is a pre filled saline syringe that comes in 5 and 10 mL with a 10mL bore on both. Even using a drawing up needle there is an 8.6% chance of contaminating your fluids. If they touch the outside of the ampoule with their needle they should be taught to throw it away and start again. I also teach to swab the injection port of the saline bag if hanging the drug even though it has been sterilized. Sterilisation only guarantees 97% kill so any bugs left alive may still be there. Dry time is key for swabbing as it is your kill time.

Hi Leanne,
I think that varies greatly! In my circumstance I was seconded to the position with no experience what so ever. Often in small rural health the person doing infection control just gets an extra hat for whatever reason. I have seen it go hand in hand with theatre roles, quality roles, ADON roles. We are extremely lucky in our region to have 2 regional consultants to play pivotal roles when this occurs and support the novices.
Most of the larger hospitals seem to have very specific requirements.Kelly

Whilst there are no exact national guidelines for educational requirement for infection prevention and control professionals, a number of bodies of work have been published in the Australian context around this (eg https://www.idhjournal.com.au/article/S2468-0451(16)30267-X/pdf).
The NHMRC national infection control guidelines (https://www.nhmrc.gov.au/about-us/publications/australian-guidelines-prevention-and-control-infection-healthcare-2019) also have quite a good discussion on the educational requirements for infection control practitioners (section 4.3) and would be a great resource for any facility who is looking to employ an infection prevention and control professional.
