    Hi ACIPC Members – I am posting this on behalf of Joslyn who contacted the office yesterday.

    If you are able to assist please contact her directly.

    I have an NDIS participant who is highly disabled with both medical conditions and disabilities including: seizures, Severe Cognitive Impairment, Short term memory deficits, Autoimmune Limbic Encephalitis, ischaemic Biliary Injury with ERCP, Depression, incontinence, Chronic Infectious Disease CRE

    The client has recently moved into a disability accommodation home but they can not have her live with any other residents due to the CRE condition and infectious nature. As the client has severe cognitive impairments, she isn’t able to adhere to safe and hygienic practices as at all times, is quite touchy and hands on with others and is not funded for individual support to ensure she does not infect the other residents in the home, who are also vulnerable with disabilities and (possibly) medical conditions.

    We need an assessment and report to be completed which takes the client’s disabilities, medical conditions and the whole situation into account and can advise us on whether she can safely live with other residents or if she requires housing with individual support without other residents. Your recommendations and professional opinion will be utilised to seek additional accommodation and support funding as she may require.

    We are able to pay for this assessment and report. Client lives in Mt Druitt NSW. Preferably assessment could be in person, but could possibly be over telehealth as well. This is needed as soon as possible.
