    Hi Jill,

    As the uptake of the COVID-19 vaccine is recommended but not mandatory I don’t believe there is a vaccination refusal form available.
    I have checked the Australian Government Department of Health website and were only able to find a consent to vaccinate form.

    Hello,
    Prior to influenza vaccinations being mandatory in aged care it was common practice to ask employees to complete a choice form if they declined to be vaccinated. This meant it forced the person to consider and make a decision.

    It was usually worded similar to:

    I__________ have chosen to / not to participate in the _________vaccination program being offered.
