I am seeking to find practitioners or groups that have a central focus on Community Care or Primary care.

Hi. I would be interested in any outcomes but not sure that I can contribute
in any way.
I work as IP&CC for a Health service that has acute, aged care, community
and general practice.

I oversee Community based IP&C in the ACT and would be happy to exchange ideas etc. I am only fairly new to the role having moved from the acute care setting.
