    Hi all,

    What your thoughts about this?

    I have noticed that the antibiotics prescribed four times a day are not being charted every 6 hours. The last dose is given at 8 p.m. and the first dose at 8 a.m., leaving the resident without a dose for 12 hours. During this time, the drug concentration drops, reducing the effectiveness of the antibiotics and potentially leading to antimicrobial resistance.
