Admission screening for Infection Risk

Hi All,

we are looking to develop a comprehensive screening tool for all patient admissions to our facility to evaluate an individuals risk of having a communicable disease or MRO (this is bigger than just COVID risk factors). Does anyone have a pre-existing tool they would be willing to share?
